Mission Statement

ForEverly Changed Inc is a non-profit that provides assistance with funeral expenses for families in West Texas who have experienced the loss of an infant in the NICU or within 30 days after NICU discharge.

Application Process

Eligibility

In order to be considered for financial assistance, the applicant must meet all of the following criteria:

Suffered the loss of an infant that was being treated in the NICU at the time of passing or up to 30 after NICU discharge.
A total household income of less than $150,000 per year.
The applicant must be the parent/legal guardian of the infant that was lost.
Live within the West Texas area.

After the application is submitted:

Requests will be reviewed by the board.
Upon approval, payments will be made directly to the funeral home, cemetery, and/or other business for related expenses up to $1500.

Funeral Expenses
Burial/Cremation Expenses

If friends and family would like to honor your baby’s memory, donations can be made to ForEverly Changed in lieu of flowers. These gifts would help provide burial gowns and financial support for other families walking this difficult journey

A copy or original bill for services rendered must accompany the Financial Assistance Application. ForEverly Changed Inc reserves the right to verify all funeral and/or related expenses.
